1. Please prepare your country report on the following topics:
   1. National Policy on Disability
   2. National Law and Regulation on Disability
   3. National Statistics on Population with Disabilities
   4. Remarkable Achievements through the Asian and Pacific Decade of Disabled Persons (1993-2002)

2. Please submit your report to APCD by 15 July 2005 by e-mail, fax, or/and post at the following address. It would be most appreciated if your report could be sent as an attached file of the email.
